The CARE-INF™ National Nursing Legacy Registry exists to permanently honor nurses who dedicated their lives to patients, families, and communities. This is not a temporary memorial. It is an enduring national archive — ensuring the profession remembers those who carried it forward.

Legacy is not transactional. Legacy is perpetual.

How it works

Three disciplined steps from submission to permanent recognition.

Submit

Provide the nurse’s name, licensure information (past or present), and basic documentation.

Verify

CARE-INF™ verifies licensure history and documentation through an integrity-controlled internal review process.

Honor

Once approved, the nurse is permanently listed in the Registry and formally recognized as part of the CARE-INF™ Legacy Archive.

Who can be listed?

  • Any U.S. nurse who held a license (active, inactive, or retired)
  • Any specialty, any state
  • Nurses who passed during service or after retirement

Permanence policy

Once verified and listed, a nurse’s name remains permanently recorded. Listing is never contingent upon financial participation.

What you’ll need

To submit a nurse, please have:

  • Full legal name (or name used professionally)
  • State(s) of nursing licensure (if known)
  • Approximate years of service (optional)
  • Proof of passing (obituary link or document upload)
  • Your relationship to the nurse (next-of-kin or authorized representative)
  • Optional: photo and short biography (up to 500 words)

Legacy Steward Circle

Families, colleagues, and institutions may choose to support nursing infrastructure in honor of a listed nurse through the CARE-INF™ Legacy Steward Circle. Participation is entirely voluntary.

Is there a fee to list a nurse?

No. Listing is permanent and never conditional upon payment or membership.

Who can submit a nurse?

Next-of-kin or an authorized representative. If you are not next-of-kin, you may still submit — we will confirm authorization during review.

What if I don’t know the license number?

Submit the nurse’s name, state, and approximate timeframe. We will attempt verification through state archives and available records.

How long does verification take?

Typical review occurs within a standard processing window. If additional documentation is needed, CARE-INF™ will follow up.

Can I include a photo and story?

Yes. Photos and biographies are optional and only published with consent.

Can a nurse be removed later?

Only in rare cases of proven inaccuracy or upon request of authorized next-of-kin.
